In general, you should be able to speak English and access information in your own language when it comes to healthcare in Sweden, but just in case, here are some handy Swedish phrases for booking the vaccine.

When you arrive at your appointment (or when booking it beforehand), you will be asked to fill out a questionnaire. In most regions it should be possible to get this in languages other than Swedish, and you can find the Swedish Public Health Agency's English translation online, but here are the main questions they will ask you:
Har du någon gång fått en kraftig reaktion efter vaccination, och behövt sjukhusvård? – Have you ever had a strong reaction after a vaccine and required hospital care?
Har du allergier som någon gång gett dig kraftiga reaktioner som du har behövt sjukhusvård för? – Do you have allergies that have ever given you a serious reaction for which you required hospital care?
Har du ökad blödningsbenägenhet p.g.a. sjukdom eller medicin? – Do you have an increased bleeding tendency due to an illness or medicine?
Är du gravid? – Are you pregnant?
Har du vaccinerat dig någon gång under de senaste 7 dagarna? – Have you received any other vaccine in the past 7 days?
