Coronavirus vaccinations started in Austria in December.
As at March 9th, the vaccinations have been largely restricted to people in risk groups.
From April however, the Austrian government expects vaccinations to be opened to the general public.
Everyone wanting to get vaccinated in Austria will need to fill in the following form.
While the Austrian government has provided English, Turkish, Bosnian, Croatian and Serbian versions of the form, the official form is only available in German.

The other versions of the form are only for translation guidance. If you fill them in and submit them, they will not be accepted.
The form asks for personal details about the person wanting to be vaccinated, along with allergy and illness history.
The final section of the form asks for a declaration of consent and explains the potential side effects that a vaccination can have.
